Some drills require physical exertion and elevated heart rate. I confirm that I understand I will be
participating in a course that will involve physical exertion and that my Primary Physician has
approved me for this level of activity.
I understand that the information provided to me surrounding and within this course is the
intellectual property of Alpine Arms and its affiliates. I agree I will not share or reproduce any of
the information shared with me to any third party without written consent from Alpine Arms.
I understand that there will be a mandatory packing list including required ammunition for this
course. I understand that weather considerations will change with the seasons and the packing
lists will reflect this. I understand that I am responsible to provide all mandatory items on the
provided packing list and ammunition for this course unless otherwise arranged with Alpine
Arms in writing. I understand that adhering to providing the required items is part of the safety
protocol and failure to provide the mandatory required items, including ammunition, may result
in my ejection from the course without refund.
I understand the inherent dangers of training with firearms and certify that I am engaging with
Alpine Arms at my own will.
I understand it is my responsibility to book any and all lodging, travel to and from the location of
this course, lunches, snacks and ample water for the required time of the course, TBA in my
welcome email. I promise to arrive at least fifteen minutes early each day of the course to be
present for any and all safety briefings. Failure to be present for safety briefings may result in
my ejection from the course without refund.
I understand Alpine Arms and its affiliates rate safety as the number one priority for participation
in any of their courses. Failure to comply with the safety standards, instruction and direction
from Alpine Arms Staff and its affiliates may result in my ejection from the course without refund.
If I am ejected from any course delivered by Alpine Arms and its affiliates for the above reasons
or any reason deemed a safety violation by Alpine Arms Staff and its affiliates, I understand I
may not be able to register with Alpine Arms for any course at any time.
I certify that I have a basic understanding of proper firearms safety and handling. If any of Alpine
Arms Staff or its affiliates deem me a safety hazard for any reason including falsifying
information on this form, I may be ejected without refund. I understand that Alpine Arms does
offer Private Instruction for Individuals and Groups for novice shooters.
Course Registration is on a first come first served basis.

I confirm all of the information I provided is current and correct. I understand the terms and
conditions of registration and participation in this course. I certify that I have read the Waiver
and Release of Liability in full have signed and understand it’s contents.

In considerations of the risk of injury while participating in
Firearms Training and Instruction (the “ACTIVITY”), and as
consideration for the right to participate in the ACTIVITY, I (the “PARTICIPANT”) hereby, for myself, my heirs, executors,
administrators, assigns or personal representatives, knowingly and voluntarily enter into this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my participation in the ACTIVITY, and do hereby release and forever discharge DV-S LLC dba Alpine Arms, located at 50 Chambers Ave, Unit G, Eagle, CO 81631, their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, for any physical or psychological injury, including but not limited to illness, paralysis death, damages, economical or emotional loss, that I may suffer as a direct result of my participation in the aforementioned ACTIVITY, including
traveling to and from an event related to this ACTIVITY.
I AM VOLUNTARILY PARTICIPATING IN THE
AFOREMENTIONED ACTIVITY AND I AM PARTICIPATING IN
THE ACTIVITY ENTIRELY AT MY OWN RISK. I AM AWARE OF
THE RISKS ASSOCIATED WITH TRAVELING TO AND FROM
AS WELL AS PARTICIPATING IN THIS ACTIVITY, WHICH MAY
INCLUDE, BUT ARE NOT LIMITED TO, PHYSICAL OR
PSYCHOLOGICAL INJURY, PAIN, SUFFERING, ILLNESS,
DISFIGUREMENT, TEMPORARY OR PERMANENT
DISABILITY (INCLUDING PARALYSIS), ECONOMIC OR
EMOTIONAL LOSS, AND DEATH. I UNDERSTAND THAT
THESE INJURIES OR OUTCOMES MAY ARISE FROM MY
OWN OR OTHERS’ NEGLIGENCE , CONDITIONS RELATED
TO TRAVEL, OR THE CONDITION OF THE ACTIVITY
LOCATION(S). NONETHELESS, I ASSUME ALL RELATED
RISKS, BOTH KNOWN OR UNKNOWN TO ME, OF MY
PARTICIPATION IN THIS ACTIVITY, INCLUDING TRAVEL TO,
FROM AND DURING THIS ACTIVITY.
I agree to indemnify and hold harmless DV-S LLC dba Alpine Arms against any and all claims, suits or actions of any kind whatsoever for liability, damages, compensation or otherwise brought by me or anyone on my behalf, including attorney’s fees and any relates costs, if litigation arises pursuant to any claims made by me or my anyone else acting on my behalf. If DV-S LLC dba Alpine Arms incurs any of these types of expenses, I agree to reimburse DV-S LLC dba Alpine Arms LLC.
I acknowledge that DVS- LLC dba Alpine Arms and their directors, officers, volunteers, representatives and agents are not responsible for errors, omissions, acts or failures to act of any party or entity conducting a specific event or activity on behalf of DV-S LLC dba Alpine Arms I ACKNOWLEDGE THAT THIS ACTIVITY MAY INVOLVE A TEST OF A PERSON’S PHYSICAL AND MENTAL LIMITS AND MAY CARRY WITH IT THE POTENTIAL FOR DEATH, SERIOUS INJURY, AND PROPERTY LOSS. The risks may include, but are not limited to, those caused by terrain, facilities, temperature, weather, lack of hydration, condition of participants, equipment, vehicular traffic and actions
of others, including but not limited to, participants, volunteers, spectators, coaches, event officials and event monitors, and/or producers of the event.

CHOOSING TO PARTICIPATE AT MY OWN WILL. I WILL NOT
HOLD DVS-LLC dba Alpine Arms, or any of it’s affiliates,
LIABLE FOR ANY SYMPTOMS OR HEALTH ISSUES THAT
MAY ARISE DUE TO MY PARTICIPATION IN THE
AFOREMENTIONED EVENTS. I WILL ADHERE TO THE
SUGGESTED SAFETY GUIDELINES WHILE PARTICIPATING
IN ANY PART OF THIS EVENT STATED AS FOLLOWS:
If you are feeling any symptoms consistent with a viral
respiratory infection (coughing, sneezing, shortness of
breath, fever, chills, aches, etc.) PLEASE DO NOT
participate.
If you have had contact with a known carrier of the
COVID-19 virus, are within 14 days of this contact and have
not tested negative for the virus yourself, please also DO
NOT participate.
If you would like to wear a face mask you are free to do so,
but we will not be requiring it of all participants/attendees
since our event is outdoors.
Please practice prudent social distancing of 6 feet between
every participant as much as you possibly can.
Please wash and sanitize hands frequently.
Avoid skin-to-skin contact with anyone - no handshakes
please.
I ACKNOWLEDGE THAT I HAVE CAREFULLY READ THIS
“WAIVER AND RELEASE” AND FULLY UNDERSTAND
THAT IT IS A RELEASE OF LIABILITY. I EXPRESSLY
AGREE TO RELEASE AND DISCHARGE DV-S LLC dba
ALPINE ARMS AND ALL OF ITS AFFILIATES,
MANAGERS, MEMBERS, AGENTS, ATTORNEYS, STAFF,
VOLUNTEERS, HEIRS, REPRESENTATIVES,
PREDECESSORS, SUCCESSORS AND ASSIGNS, FROM
ANY AND ALL CLAIMS OR CAUSES OF ACTION AND I
AGREE TO VOLUNTARILY GIVE UP OR WAIVE ANY
RIGHT THAT I OTHERWISE HAVE TO BRING A LEGAL
ACTION AGAINST DV-S LLC dba Alpine Arms FOR
PERSONAL INJURY OR PROPERTY DAMAGE.
To the extent that statute or case law does not prohibit
releases for negligence, this release is also for negligence
on the part of DV-S LLC dba Alpine Arms, its agents and
employees. In the event that I should require medical care or treatment, I agree to be financially responsible for any costs incurred as a result of such treatment. I am aware and
understand that I should carry my own health insurance. In
the event that any damage to equipment or facilities occurs
as a result of my or my family’s willful actions, neglect or
recklessness, I acknowledge neglect or recklessness. This
Agreement was entered into at arm’s-length, without duress or coercion, and is to be interpreted as an agreement between two parties of equal bargaining strength. Both the Participant, and DV-S LLC dba Alpine Arms agree that this Agreement is clear and unambiguous as to its terms, and that no other evidence will be used or admitted to alter or explain the terms of this Agreement, but that it will be interpreted based on the language in accordance with the purposes for which it is entered into. In the event that any provision contained within this Release of Liability shall be deemed to be severable or invalid, or if any term, condition, phrase or portion of this agreement shall be determined to be unlawful or otherwise unenforceable, the remainder of this agreement
shall remain in full force and effect, so long as the clause
severed does not affect the intent of the parties, If a court
should find that any provision of this agreement to be invalid or unenforceable, but that that by limiting said provision it would become valid and enforceable, then said provision shall be deemed to be written, construed and enforced as so limited.
